Columbia University Medical Center

Enrollment in our elementary- and intermediate-level courses is reserved for undergraduate and graduate students (e.g. Graduate School of Arts and Sciences, School of International and Public Affairs) who need these courses to fulfill degree requirements.

If spots are available, Columbia University Medical Center (CUMC) staff and students must complete the following procedures:

  1. If you have not taken the prerequisite at Columbia or Barnard, take the online placement test: click here for instructions for taking the exam and click here to take the exam
  2. Save a screen capture of your results and submit them to your instructor on the first day of class.
  3. Keep perfect attendance (no absences) during the Change of Program period.
  4. Download the University’s Registration Adjustment Form (click here to download)
  5. Bring a copy of the Registration Adjustment Form to the instructor to obtain her/his approval.
    **NOTE: Staff in the Department of Latin American and Iberian Cultures cannot sign a Registration Adjustment Form on behalf of an instructor
  6. Obtain the instructor’s signature / approval
  7. Submit the signed Registration Adjustment form to the CUMC Office of Registrar Services (Registrar and Student Account Services 650 West 168th Street, 141 Black Building, New YorkNY 10032: See Google Map)
