“LifeNet” is the access point for Mobile Crisis Team Services for New York City.

1. During the hours of 9:00am – 4:30pm Mondays through Fridays (excluding holidays) providers must use the “Fast-track” Process to make referrals.
2. Outside of the Fast-track hours, on weekends, and holidays you must call 1-800-LIFENET (543-3638) to make a MCT referral.

Fast Track Process

• Go to http://lifenet.nyc/make-mct-referralto download and complete the Fast-track form.
• Fax the form to LifeNet at 866-314-7707.
• LifeNet will contact you after the referral is submitted.
